Referrals and Insurance


Office Visits

Our managed care patients cannot be referred to our specialist without first being evaluated by their primary care physician. The insurance companies mandate this and we ask for your cooperation in providing us with the necessary referrals at the time of your office visit.

To reduce bookkeeping and billing costs, we request payment at the time of the visit. Our billing office will submit a completed claim form to your insurance company for your reimbursement. If you have Medicare, you will be expected to pay 20% of the office fee. If you belong to one of the managed care plans with which we participate, you are responsible for your co-payment at the time of the visit. A $15.00 surcharge will be added to all co-payments that are not paid at the time of service.

Hospital Patients

Tisch Hospital is where all of our patients at New York University Medical Center are treated and where each of our physicians has admitting privileges.

The emergency room entrance is on First Avenue at 33rd Street.

If you are hospitalized and want information given to your family, please identify one family member you authorize us to share information with.

Please share with us any changes in address, telephone numbers, insurance, etc. This will help us keep your records up to date and make it possible for us to reach you when needed.
