Help Downloading Forms

It is recommended that you download the forms in Microsoft Word format. When prompted to "open the document in the browser" or to "save it to the disk", Select "save it to disk". This will allow you to view the file properly.

Using MS Word will allow you to enter your information into text fields (shaded in gray). To move from field to field simply press the “tab” key or click the field with your mouse.

PDF format does not allow you to enter information into text fields. We are working on developing PDF documents in form mode, but they are not available at this time

Follow these instructions to download any IRB forms to your hard drive:

To download a form to your hard drive in Netscape and Explorer:

1. Click on the form that you want to download. MS Word is strongly recommended.

2. Two possible things may occur:

You will see a dialog box that will ask you to “save” the form, or “open” it

Or you will see a dialog box that will ask you where you would like to save the document.

3. It is best just to save it to its designated file location later. Netscape will launch Word and open the document.

4. After you save the form, select a location to save the file (make sure you remember where you have placed the document) and click ‘Save’.

5. Your form is now saved in the designated folder you have chosen. You can go to that location and open the document anytime.

****If you attempt to open the document in your browser and only see weird characters, you will need to save the document first. Then open the document in Microsoft Word.
