Programs
The Lower New York Consortium supports programs for women, youth and families. Our network of providers in lower Manhattan and Staten Island offer family-centered, case managed HIV/AIDS care, as well as social services, linked to community case management so that you don't have to tell your story over and over again to many different providers.
Programs for Women
Specialized services cater to pregnant women, those at high risk and women of color.
Project YOUTH
Serving children from ages 13 to 24, Youth Outreach United for Teen Health aims to reduce risk levels through education and to increase access to services.
Case Management and Care Coordination
Case managers and clients work together to identify and address health and social issues that affect the client’s ability to access resources and services needed to improve and maintain their quality of life. Another Consortium goal is to increase case management collaboration between the community and clinic.
Clinical Trials
Our various affiliations make clinical trail referrals more accessible to our patients.
